In-Network Insurances & Billing
Purpose built is currently in-network with: Blue cross of idaho, select health, pacific source, and st. Luke’s Health plan.
After booking an appointment an insurance verification form will be emailed to you. As a courtesy service we will verify insurance coverage and details.
these benefit estimations do not guarantee coverage and final cost will be determined once your insurance company has processed claims and a patient responsibility is determined.
Patient will be responsible for co-pays at the time of service.
when booking online there will be cash-based options connected to each service, you will not be billed this amount if you are utilizing an in-network insurance plan.
credit card information is required during booking, your card will not be charged until the session is over and coverage/bill is determined.
Cancellation policy - if you cancel or reschedule, we require a 24 hour advance notice. If you cancel or reschedule within 24 hours, you will be charged a $50.00 charge.
Out-Of-Network Insurances & Superbills
PURPOSE BUILT IS CURRENTLY OON WITH ALL OTHER INSURANCES.
Out-of-network (OON) means we don’t have a contract with your health insurance plan BUT we do provide superbills for people with out-of-network benefits and we do accept HSA/FSA.
Superbills are medicals receipts that we provide you when you pay upfront. You can use this receipt as a way to request reimbursement directly through your insurance depending on your specific insurance benefits.
